Seminar title: Shifting sands: understanding and supporting medicines management work at end-of-life

We would like to invite you to a seminar which will draw on learning accumulated from three research studies, all large multisite investigations, conducted over the last eight years to illustrate the work involved in managing medicines for individuals living at home towards the end-of-life. We will illustrate experiences from multiple perspectives: patients, carers and community-based healthcare professionals (specialist and community nurses, primary care and community pharmacists, and general practitioners); and highlight conceptual modelling that demonstrated for the first time the reality of supported self-management in this context. The seminar will close by raising the question what good medicines management at end-of-life should look like and what might be required of service providers if this is to be turned into reality.
